Disease Management is a program for people with or at risk for Diabetes, Heart Failure, Chronic Obstructive Pulmonary Disease, Asthma, Hypertension and Hyperlipidemia. The goal of this program is to help you manage your risks and establish healthy lifestyle behaviors.
As a participant in this program, you will receive educational materials, telephone calls and/or in-person meetings with a health coach. A health coach will assist you in developing an action plan and help you understand and manage your medications. Your health coach will also keep your healthcare providers informed of your action plan, goals and progress. The information you receive through this program does not replace your provider’s care. Please discuss any questions or concerns you may have with your provider.
Your membership in the Disease Management program is voluntary. If at any time you wish to stop participating, opting out is simple. To opt out of the program, please contact the Disease Management staff at (864) 522-2060 or firstname.lastname@example.org.
The Disease Management program is not an emergency provider. If you need urgent care, immediately call your physician, go to an emergency department or call 911. For non-urgent care, please contact the Disease Management program at (864) 522-2060.
