Disease Management

MyHealth First Network offers a disease management program for all eligible health plan members who are identified or at risk for Diabetes, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease, Asthma, Hypertension and Hyperlipidemia. This program works collaboratively with the member’s treating providers to identify opportunities for prevention or intervention in an effort to provide well-coordinated care.

Members who participate with this program will work with a health coach to develop a plan of care based on evidence-based guidelines and interventions specific to their identified condition or risk, as it relates to screenings, immunizations, self-management education and support, medication adherence and goal setting. In addition, health coaches will assist patients with referrals to weight management, smoking cessation, alcohol and depression programs and support groups. Health coaches will engage members through telephone calls, in person meetings, and distribution of educational materials.

As a treating provider you can expect to receive the member’s disease management care plan, along with scheduled and unscheduled updates on member enrollment, opportunities for collaboration and member’s progress, via messages in the electronic medical record. Likewise, you will be able to provide any feedback to the health coach. Unscheduled communication between the health coach and treating provider will occur if there are immediate concerns about the patient’s safety or well-being, if the member presents a clinical question that requires physician input, if there are questions or concerns about patients medication list or any other reason identified as requiring a treating provider’s input.
