Care & Condition Management

Care and Condition Management services are designed to drive patient engagement and coordinate care across the entire continuum. By leveraging evidence-based best practices, our programs ensure that the right care is delivered in the right setting through care plan design, implementation and monitoring. This approach leads to improved health outcomes, more positive experiences and reduced overall cost of care.

Our care and condition managers received Case Management Accreditation from the National Committee for Quality Assurance (NCQA) and a Certification for Disease Management. Earning NCQA’s case management accreditation is an indication that a Case Management program is dedicated to care coordination, patient-centeredness and improving quality of care: getting the right care to patients.
